Job Title: Associate Director of Integrated Care

Schedule: Full-Time | Tuesday–Saturday
Compensation:$140,000–$170,000 annually + bonus
A premier senior living community in New York City is seeking an experienced clinical leader to serve as Associate Director of Integrated Care. This leadership role oversees resident care and clinical service delivery within a combined assisted living, memory care, and enhanced care environment. Reporting to senior leadership, this position plays a critical role in driving quality outcomes, regulatory compliance, operational efficiency, and exceptional resident experiences.
Position Overview
The Associate Director of Integrated Care is responsible for developing and executing clinical strategies, overseeing nursing and ancillary services, ensuring regulatory compliance, and supporting high-performing care teams. This role partners cross-functionally with community leadership to align care delivery with operational goals while maintaining the highest standards of safety, quality, and resident satisfaction.
Key Responsibilities
- Develop and lead clinical training programs to ensure staff competency and excellence in care delivery
- Assess resident, family, and staff needs to develop strategic and operational care plans
- Oversee nursing, ancillary services, provider partnerships, and quality assurance initiatives
- Ensure compliance with state regulations, policies, and internal standards
- Address and resolve regulatory findings with corrective action planning
- Monitor departmental performance using KPIs and business analytics
- Assist in developing budgets, staffing models, and operational strategies
- Conduct audits to maintain clinical and regulatory excellence
- Collaborate with department leaders to ensure coordinated, resident-centered care
Qualifications
- Bachelor’s degree required; NY-licensed Registered Nurse (RN) in good standing
- Advanced degrees or certifications preferred
- Minimum 2+ years of leadership experience
- Experience in senior living, long-term care, skilled nursing, or home health
- Minimum 2+ years serving older adults with complex medical needs, including dementia care
- Strong knowledge of staffing models and clinical operations
- Proficiency in Microsoft Office and electronic health record systems
- Must meet health and compliance requirements
